📌 2021 Update: This calculator uses the revised CKD-EPI equation that removes the race coefficient per NKF/KDIGO 2021 recommendations. The race-neutral formula provides more equitable estimates across all populations.

📖 Understanding the Formulas

Cockcroft-Gault

Best for: Drug dosing in pharmacy practice. Most medication package inserts reference this formula.

Limitation: Less accurate in obesity, elderly, and extremes of muscle mass. Assumes stable kidney function.

Note: If height is not provided and IBW/ABW is selected, height will be estimated from weight.

MDRD

Best for: Estimating GFR for CKD staging. Commonly used in clinical labs.

Limitation: Less accurate at higher GFR values (>60 mL/min). Largely superseded by CKD-EPI in modern practice.

CKD-EPI (2021 Race-Neutral)

Best for: Most accurate GFR estimation across all levels of kidney function. Preferred by KDIGO guidelines.

Update: The 2021 version removes the race coefficient for more equitable care. This is the recommended formula for clinical use.

⚠️ Clinical Disclaimer: This calculator is for educational and clinical reference purposes only. Results should be interpreted by qualified healthcare professionals in the context of the complete clinical picture. Always consult current drug information and clinical guidelines for specific dosing recommendations. Metformin may be used with careful monitoring between 30-45 mL/min per current guidelines.
